To obtain this achievement you simply need to play through the start of the game. You will take a ride in the back of a jeep to a hotel in Africa. At the hotel you will be stricken with malaria and your objective is to leave the town. Except there is a massive firefight once you hit the streets which you can not survive. Once you are downed then achievement will pop.
